Long Term Care

Long term care provides maximum comfort and attention for those residents who no longer can benefit from lower levels of care and require closer monitoring.
24 hour nursing care and a physician available daily assures the highest level of medical attention. Residents also in-house physician at health care facility in New York benefit from complete therapy, treatment and rehabilitation services including IV therapy, pain management, wound care and dementia care.

We also maintain transfer agreements with area hospitals for in-patient and outpatient services.

Short Term Care

The goal of short term care is to provide care and rehabilitation services to those discharged from a hospital who may still not be well enough to either fully recover at home or in an assisted-living environment. Middletown Park Rehabilitation and Health Care Center utilizes a variety of recovery strategies to return patients to their maximum potential.

The goal is to improve an individual's health and help them achieve the highest possible

level of functioning until they are ready to return home or to a lower level of care.
