History And Mission   Middletown Park Rehabilitation and Health Care Center was formerly known as Park Manor. Park Manor was founded in 1971. After five years of planning and obtaining required approvals, construction was started in September 1976 and completed May 1978. The facility was initially an Intermediate Care Facility. In 1984, eight-five of the one hundred and thirty beds were converted to Skilled Nursing in order to meet the increased medical needs of the residents. In 1995, a one hundred and twenty bed, state of the art addition was built onto the existing facility. In 2002, the facility changed its name to Park Manor Rehabilitation and Health Care Center, to reflect the changing nature of services provided to residents and the community. Park Manor continued its over thirty year history of excellent care and personalized service as one of the largest and most progressive health care providers in Orange County. In March 2010, Park Manor Rehabilitation and Health Care Center underwent a change in ownership. We are now operating and known as Park manor Acquisition II, LLC, (dba, Middletown Park Rehabilitation and Health Care Center).
