HOW DO I APPLY FOR ADMISSION?
A. Application forms may
be picked up at the Middletown Park Rehabilitation and Health Care Center Reception Desk or will be mailed to you at your request.
The application form must be completed by you; the yellow Medical
History and Physical Examination form must be completed by your
physician. These forms must be returned prior to admission.
(A physical examination is required prior to admission). Please
contact the Admissions Office for Information about admission
requirements and procedures.
If you are applying
for admission and you are living at home, the New York State
Department of Health requires you to have a Home Assessment.
The Home Assessment determines the level of care you require.
You must call the Public Health Department in the county in
which the applicant resides in order to schedule the Home
Assessment. There is a cost involved for this service that
you pay directly to the Public Health Department. We need
to have a copy of the Home Assessment for our files. If you
are in the hospital, the Hospital Case Manager will assist
you in forwarding the PRI/Screen to Middletown Park Rehabilitation and Health Care Center. Upon completion of your application
for Admission, Home Assessment, (PRI and Screen) this information
will be processed for review by our Admissions Committee.
Incomplete Applications cannot be processed. A personal interview
is required.

MAY I VISIT Middletown Park Rehabilitation and Health Care Center BEFORE I AM ADMITTED?
A. Yes. Our facility is
open to visitors from 10:00 a.m. to 8:00p.m. daily. The Business
Office is open from 9:00 a.m. to 5:00 p.m. Monday through
Friday. During those hours someone will be available to answer
your questions about Middletown Park Rehabilitation and Health Care Center. It is preferable that appointments
to tour the Facility be arranged by calling the Admissions
Office so that we may devote the time necessary to fully explain
our programs and services.

SHOULD I BRING MY MEDICATION WITH ME?
A. No. Our policy states
that outside medications may not be brought into the facility.
When you arrive for admission, the doctor will order your
medication directly from the pharmacy. This policy was developed
for your safety and protection. Your cooperation is essential
in this matter.
Q.
MAY I MAKE AND RECEIVE PERSONAL TELEPHONE CALLS?
A. Of course. A public
telephone is available in the main lobby and on each unit.
Each room is equipped for private telephones and arrangements
can be made with the telephone company if at some time the
resident or family would like one installed at their expense.
Q.
WHERE MAY I HAVE MY HAIR DONE?
A. Our complete Barber
and Beauty Shop is available to all residents. Payment is
made from resident's personal accounts unless other arrangements
have been made by the family or resident.
Q. ARE ANY RELIGIOUS SERVICES HELD
AT Middletown Park Rehabilitation and Health Care Center?
A. A variety of religious
services are held on a regular basis at Middletown Park Rehabilitation and Health Care Center for residents
of all faiths. Local clergy also visit the facility regularly.
The schedule for religious services is posted in the facility
every month.
