Questions And Answers For Admission

Q. HOW DO I APPLY FOR ADMISSION?
A. Application forms may be picked up at the Park Manor Rehabilitation and Health Care Center Reception Desk or will be mailed to you at your request. The application form must be completed by you; the yellow Medical History and Physical Examination form must be completed by your physician. These forms must be returned prior to admission. (A physical examination is required prior to admission). Please contact the Admissions Office for Information about admission requirements and procedures.

If you are applying for admission and you are living at home, the New York State Department of Health requires you to have a Home Assessment. The Home Assessment determines the level of care you require. You must call the Public Health Department in the county in which the applicant resides in order to schedule the Home Assessment. There is a cost involved for this service that you pay directly to the Public Health Department. We need to have a copy of the Home Assessment for our files. If you are in the hospital, the Hospital Case Manager will assist you in forwarding the PRI/Screen to Park Manor Rehabilitation and Health Care Center. Upon completion of your application for Admission, Home Assessment, (PRI and Screen) this information will be processed for review by our Admissions Committee. Incomplete Applications cannot be processed. A personal interview is required.
